Insurance 

Dr. Cole-Straub is a participating provider with the following insurance companies: Aetna and United.

For insurance companies in which Dr. Cole-Straub is not a participating provider, she is considered 'out of network.' Dr. Cole-Straub can provide clients with documentation that may be used to file 'out of network' with insurance companies. Dr. Cole-Straub's fees as are follows: Initial Consultation (60 minutes):  $250 and Individual Therapy (45-50 minutes): $175.
